New York State Birth Certificate Application

NY Birth Certificate Types
You can order either a short form or a long form NY birth certificate. Both types are certified documents. However, depending on the reason you need your certificate copy, you may need a short or long version.

New York short form birth certificates are also sometimes called certified transcripts of birth. They only list basic details, including your name, gender, date and time of birth, and your parents’ names.

New York Birth Certificate Cost
For births in New York State (outside New York City), the cost of a birth certificate is $30 per copy, whether you request a certified copy or an informational copy. Informational copies are limited to older records — only certificates that are at least 75 years old and where both parents named on the record are deceased.
If you require an extra copy or your New York State birth certificate, you may request them by paying $30 for each copy you need.
For mail requests, payment is made by check or money order payable to the New York State Department of Health. Cash is not accepted for mailed applications.
New York Birth Certificate Processing Times
