Who Can Request a Florida Birth Certificate

You need to be eligible to request some types of vital records. State laws allow Florida authorized birth certificate copies to be issued to any of the following:

  • The certificate holder, or their parent or legal guardian
  • A child, grandparent, grandchild, sibling, spouse, or domestic partner of the certificate holder
  • A member of a law enforcement agency or a representative of another governmental agency who is conducting official business
  • A party entitled to receive the record as a result of a court order, or an attorney or a licensed adoption agency seeking the birth record

If the applicant is not one of the above, the request must be accompanied with a notarized Affidavit to Release a Birth Certificate (DH Form 1958). This must be signed by an eligible person and have attached a copy of valid photo identification of both the person authorizing release and the applicant.

Florida Birth Certificate Apostille

The apostille is issued by the Secretary of State and it’s attached to an original document to verify that it’s legitimate and authentic. This means your birth certificate can be accepted in other countries who are also members of the Hague Apostille Convention.
