A long form birth certificate may refer to a certificate with parental information or a certified copy of birth registration. This official document is issued by the state’s health department. A government agency might ask you to submit a long form birth certificate for identification purposes.

The long form birth certificate is generally an image of the original record and contant all the legal information of the person, including the hour of birth and the name and signature of the attendant.
