What information am I required to provide to get my birth certificate copy?

We have simplified the application process to obtain a birth certificate. To apply for a certified copy of a birth certificate you will need to complete our online form and provide as much information as possible so that the Health Department is able to locate the birth certificate. Information required may include:

  • State of birth
  • Full name at birth
  • Date and place of birth
  • Sex
  • Full maiden name of mother
  • Full name of father
  • Your name
  • Your relationship to the person whose certificate you are requesting
  • Address where you want to receive the certificate
  • Reason for requesting the certificate

This is the information required in most cases, however, certain states may need the registrant to disclose more personal or family data in order to have a new certified copy of their birth certificate issued.
